When a practice administrator asks ChatGPT "What is the best EHR for a 15-provider multi-specialty group?" the AI returns a recommendation that shapes a purchasing decision worth $200K-$2M over the contract lifetime.
We tested 20+ EHR and healthcare IT queries across ChatGPT, Perplexity, Gemini, and Claude in April 2026. The results show a market with clear leaders at the extremes (Epic for hospitals, athenahealth for small practices) but a wide-open middle. Specialty-specific, size-specific, and function-specific EHR queries are underserved across all AI platforms.
Healthcare professionals are among the earliest and heaviest AI adopters. Clinical decision support via AI is now standard. That same trust in AI extends to technology evaluation. The EHR vendor that shows up when a CMIO asks AI for recommendations captures an evaluation that may not come around again for 5-10 years.

Pattern: Specialty-specific EHR queries are where the opportunity lives. For some specialties (physical therapy, behavioral health), niche EHR vendors dominate. For most specialties (cardiology, OB/GYN, orthopedics), AI returns generic results because no vendor has created specialty-specific content. The first EHR vendor to publish "EHR for [Specialty]" pages with specialty-specific workflows, compliance requirements, and feature comparisons will own those citations.

Pattern: Perplexity cites KLAS Research heavily for EHR recommendations. KLAS is the gold standard for healthcare IT vendor ratings. EHR vendors with strong KLAS scores get a significant citation boost. Healthcare IT News and Becker's Health IT are also frequently cited sources.
How EHR vendors can capture AI citations
Create specialty-specific pages. "EHR for [Specialty]" with specialty-specific workflows, template screenshots, compliance requirements, and customer examples. Target 5-10 specialties where your EHR has customers.
Create practice-size-specific pages. "EHR for Solo Practitioners," "EHR for 10-50 Provider Groups," "EHR for Community Health Centers." Size determines feature requirements and budget constraints.
Publish comparison content. "[Your EHR] vs athenahealth," "[Your EHR] vs eClinicalWorks." Include specific comparison points: interoperability, billing integration, specialty support, pricing models.
Leverage KLAS data. If you have strong KLAS scores, feature them prominently on your site with structured data. Perplexity cites KLAS directly.
Create function-specific content. "EHR with Telehealth," "EHR with Revenue Cycle Management," "EHR with AI Clinical Decision Support." Function-specific queries are growing rapidly.
